Application Procedures

Applications and enrollment packets for the 2014-2015 school year will become available on Tuesday, January 21, 2014. Spaces are limited, are on a first come, first serve basis, and do fill up quickly. To request admission materials, please click here.


3 or 4 Year Old Kindergarten

Students entering 3 or 4 year old kindergarten must be age 3 or 4 by September 1, 2014. Additionally, all students must be fully potty trained by the first day of school. 

1. Request Admission Materials
Request materials from the Admissions Office by clicking here or by calling (818) 340-6131, ext. 225. 

2. Apply
Submit the application, enrollment fee (check or cash), and a SMART Tuition form to the Financial Office. A certified copy of your child's birth certificate is required at the time of enrollment.

3. Acceptance
Once you submit your application, your child is automatically accepted, provided that spots are available. Additional enrollment materials and requirements must be submitted no later than August 1, 2014. To view a list of enrollment materials and requirements, please click here.


5 Year Old Kindergarten

Students entering 5 year old kindergarten must be age 5 by September 1, 2014.

2. Apply
Submit the application, enrollment fee (check or cash), and a SMART Tuition form to the Financial Office. A certified copy of your child's birth certificate is required at the time of enrollment.

3. Testing
All students entering K5 must take and pass a 1 hour entrance test. After receipt of application, new students will automatically be placed on the testing list. Testing dates and times are available by clicking here

4. Acceptance

1st - 12th Grades

1. Request Admission Materials

2. Apply
Submit the application, enrollment fee (check or cash), and a SMART Tuition form to the Financial Office. A certified copy of your child's birth certificate is required at the time of enrollment.

3. Testing
All entering 1st -12th grade students must take and pass the Stanford Achievement Test at grade level. After receipt of application, new students will automatically be placed on the testing list. Testing dates and times are available by clicking here.  

4. Letter of Recommendation
A satisfactory letter of recommendation from your child's previous teacher or principal must be received prior to acceptance. Please use Faith Baptist's provided standard form inside the enrollment packet. You may also download the form by clicking here.

5. Final Report Card
A full year's report card with passing grades must be received prior to acceptance. A student with grades resulting in D's or F's will not be accepted.

6. Acceptance
Final acceptance will be granted after your child passes the Stanford Achievement Test and the school receives a satisfactory letter of recommendation along with an acceptable full year's report card.
